First-time Social Security numbers

There have been several changes and additional requirements for those who wish to apply for a first time social security number. Therefore, the passport section will no longer be taking social security applications as part of the request for registration of a child. When you have received the Consular Report of Birth Abroad, and/or passport, you may apply for a SSN for your child with the Federal Benefits Unit (go straight to window 7 in the Consular Section). social We have listed the ORIGINAL documents you will need to apply for your child's security number:

  • Clear and legible application form, completed and signed (available at the Embassy and/or at the SSA web site: www.ssa.gov)
  • Israeli Birth Certificate
  • Proof of U.S. citizenship for the child (either a signed U.S. passport, Naturalization Certificate or Certificate of Birth Abroad)
  • Identification document for child (signed U.S. or Israeli passport (unexpired). Only if unavailable, we can accept a recent document showing child's name and date of birth, or child's name and the parent's names.
  • Please note that this should be an immunization card, well baby card, health certificate, letter from a doctor or child's Israeli identification document.
  • Any official name change document, if applicable
  • Identification for parent signing the form
  • For first time U.S. born applicants, documentation proving Israeli residence is required (e.g. primary and high school records, immunization cards, army records, etc, with translation. In this case, the U.S birth certificate is required.

Please note that all U.S. passports must be signed. N.B. First time social security applicants age 12 or over MUST appear in person. Those 11 and under are not required to appear, although everyone MUST appear for passport and report of birth requests.
