Lost or Stolen Passports
 

Note:  Please enter your Social Security Number in the correct field on your passport application.

If your passport is lost or stolen, you are advised to report the loss/theft to the local police in the area in which it occurred. In order to replace the passport, please make an appointment through the following website http://israel.usembassy.gov  (it will be necessary for you to appear personally at the Embassy). You should submit your application for a replacement passport as soon as possible after the loss or theft is discovered.

Application Requirements

Please bring an original of the following to the Embassy for your appointment:

1.Proof of U.S. citizenship:

·  an original U.S., Consular Report of Birth or foreign birth certificate which contains both parents’ names (abstracts of birth, also known as birth abstracts, and hospital birth certificates are Not accepted);

· naturalization certificate;

· certificate of citizenship;

· consular report of birth abroad; or

·  an expired passport;

1.Photocopy of all documents including all passports and proof of citizenship. You do not need a photocopy of the application.

2.Proof of Identity

· a valid passport of any country;

· an Israeli ID card (Teudat Zehoot) with photograph;

· a valid U.S. or state government ID card; or

· a U.S. military ID card; or

· a drivers license.

3.Passport application form DS-11(pdf - 1000kb):

· Please ignore pages one and two of the instruction pages, as they do not apply when applying for your passport in the Israel. Follow the instructions on this webpage.

· Form DS-11 must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet. For tips on completing the DS-11,

5.One passport photograph, 5x5 centimeter: The Embassy will reject any photographs that do not meet the specified requirements. Vending machine photographs and photographs printed on a home computer are unacceptable. Follow this link to photo requirement specifications;

6.Fees: The application fee for an applicant under the age of 16 is U.S. $105.00; aged 16 and over is $135.00. Accepted payment methods are:

  • Cash - either dollars or shekels; or
  • Most international credit cards.
  • Note: The Embassy is not authorized to accept personal checks.

7. Envelope:

New passports must be sent back to applicants once they are processed.  In order to receive your new passports, you must either:

 

1.If applying for a child under the age of 14, you will also be required to follow additional instructions.

Processing Time

An application for a full-validity passport will be sent to the United States for printing with a secure, photo-digitized image of the bearer. Passport times vary depending on workload. Please allow 3-4 weeks for the application to be processed.

 

 

  • bring a self-addressed, stamped envelope with at least 16.70 NIS of postage for registered mail, or
  • request a delivery service envelope at the Embassy. The fee for this service is 40 NIS, payable to the delivery company by phone, by credit card.
