Change of Name Procedure

If the name you are currently using differs from the name on your most recent passport, you must submit either:
    1) An original marriage certificate or

    2) a court order showing the change of name or

    3) an original document from the Ministry of Interior showing your official name change.

    4) three current pieces of identification with pictures showing the use of your new name.

NOTE: The name change document MUST bear the official seal of the issuing authority. Uncertified copies or notarized documents cannot be accepted.
